Payment must be made within 24 hours of the submission of this form. DownDog Yoga will only reserve your interest till that time. After 24 hours of the submission of this membership form, if no payment is received by DownDog Yoga, you will be required to complete a new membership form and confirmation of requested membership/workshop/private classes/be-spoke outdoor yoga session will be subject to availability.

I understand that it is my responsibility to consult with a physician prior to participating in any physical fitness program, including Yoga, and to inform the Yoga instructor of any medical conditions or physical limitations, before the start of the private or group Yoga session,
I agree to assume full responsibility for any risks, injuries or damages, known or unknown, that may occur as a result of my participation in classes at DownDog Yoga India Pvt Ltd (DownDog Yoga) or at locations where DownDog Yoga will be conducting Yoga sessions.
